Nice exercise.  I suggest adding a few more pages with the answer key.  (or better yet, upload it in a second file).  They're not that tough to figure out, but since this is a quiz, the people taking it will get them wrong at times.  The answer key will help them spot and understand their errors.

Great quiz, just one comment...it would be better to use the center of the circles rather than the "nearest edge" as the point to measure form to get across the point about it being the "hips" (centerline) of the player we judge form, not the extent of the track they are covering ( eg extended arms).
This sort of thing is really helpful thanks to all involved in making it and getting it public
