Expulsion Form help

Hello everyone. I'm currently looking at the Expulsion form portion of the stats book for the first time in my career. Can someone who has worked with it before explain it to me? I've read the instructions at the top, and it seems straightforward enough, but there are more squares here than there are titles or descriptions as to what should go in each of them.

For instance, a box with "Referee initiating the expulsion," which is straight-forward enough. However, there is one immediately beside it with no title or description. What goes in there?

Further, there doesn't seem to be enough space to even provide a basic description of the expulsion...

I believe you're expected to fill in your name and position in that bout. Using the back of the sheet, or additional sheets, is acceptable for writing the description.

It looks like the blank boxes are just a formatting decision, not a content box.  This form is designed to be printed off and brought to bouts to be filled out in case the worst happens and someone effs up just that badly.  It's not something that's going to show up in the Bout Stats (or muck up the bout stats if you 'fill it out wrong'), so use the blank boxes as more space to write what happened.

A foul out and an expulsion are distinctly different things.  Yes, they both result in the skater leaving the game but the foul out is removal by commiting too many actions that fall within the acceptable range of the unacceptable.

An expulsion means the person did something outside of the normal unacceptable.  A skater can be expelled off of a no impact event and have zero penalties in the bout.  They commited an action that in words of gross misconduct was:

[rule]an indiscretion so serious that it justifies the instant expulsion of a skater, even on the first occurrence.[/rule]

Anything that meets that scale should be something incredibly atypical in a bout and worth having a record kept of exactly what happened that led to the head referee deciding that someone had to be removed form the game immediately.
