In a bout last night, between two jams, a team had 2 pivots in the penalty box.

So, yeah, clearly we missed something in the previous jam.

My question is...

How does one rectify this situation?

Determine which pivot was "second" and give her cover back to the team, between jams?
Issue a penalty between jams?
Do nothing and issue the penalty if there's still 2 pivots at the start of the next jam?
Something else?

[rule]9.2.1.1.2 If the jam starts with too many skaters and the extra skater cannot be pulled, the referee must stop the jam. The team must be penalized according to Section 6.13.6.

9.2.1.1.3 Referees do not warn teams when too many skaters line up on the track.[/rule]

If you go by this, you would start and call off jams until the "wrong" Pivot's penalty time has expired. You could be handing out a dozen or more Illegal Procedure Majors to the team. (until they realise they have too many Pivots and remove one cover which they may because one of the skaters in the box is no pivot and therefore not serving time as pivot... etc.) This is an insane loop and in no way equal to the infraction that occured.
I would ensure that there is one pivot and go on with the game. You could give the team a IP minor for too many pivots (and the jam not being called) but this would lead you back into the "loop".
I would just remove the 2nd cover from the box, get the team captains and explain what happened and what I decided and if that happens to the other team it would be handled just the same.

I'd think this is still within the limits of referee's discretion. (my personal humble opinion and approach)

Can you issue the IP for too many pivots between jams?  Obviously the team had committed an illegal procedure in order to get two pivots to the box, but if everyone missed it, can it be penalized between jams when the team could field ten pivots legally since it's between jams and doesn't count until the jam starts?

I would issue the minor. Jam wasn't called, so no major, but there was verifiably too many skaters (pivots). Figure out who and give the minor to them.

Beyond that, I wouldn't consider the "loop" and issue more penalties. That "extra" skater may have identified herself as a pivot, and though she was not removed, I also would not consider her a pivot in the box (we penalize player and position, but she's not really a pivot, so no "loop" as you say). I'd allow removal of the cover between jams.

Not one of those explicitly spelled out things, so this is just guidance. It's what I believe I'd have done in the spur of the moment.
